Health Information Partnerships for Brooklyn

Brooklyn Public Library BPL will develop and implement a 5-month project of expert-led health information programs targeted to vulnerable populations with identified health challenges. Addressing known social determinants of health specific to Brooklyns varied communities, the programs will address the topic areas of: 88tal Health, 102, Health Insurance and Access to Care Navigation, Nutrition, and Positive Aging. The 60 planned programs will increase 525 patrons use of reliable health information and resources, and encourage them to embrace wellness, make healthy choices, and manage illnesses and chronic conditions effectively. BPL will partner with organizations in the community that have specific clinical and public health expertise to tailor messages and content to patron needs. Programs will be conducted by one or more of BPL's 20 community partners and presented virtually via Zoom and other accessible platforms, or, if conditions allow, in-person at one or more of BPLs 60 neighborhood locations.
