Improving Access to Chronic Disease Information and Education to Decrease Preventable Death in the Lower Mississippi Delta


Sharkey Issaquena Community Hospital in Rolling Fork, MS, will add educational and informational outreach efforts to include chronic diseases of type 2 diabetes, hypertension, and cardiovascular disease. These efforts will concentrate on prevention through education, physical fitness, and healthy eating habits. This will bring hands on, patient centered training, cooking instruction, and subject matter experts to reduce the prevalence of mortality related to chronic illnesses in the counties of Sharkey and Issaquena. Additionally, one on one education and evaluations will be offered to monitor health markers to access success of project and participant responsiveness.
