2024 NNLM Active Member Survey

Every couple of years, the NNLM reaches out to its member organizations for feedback on its membership, its value and to learn more about unmet needs. If your member organization was actively engaged with the NNLM from May 1, 2023 to April 30, 2024, you will soon be receiving an email invitation to take part in the online 2024 NNLM Active Member Survey. Your responses will be used to inform decisions about future programs and services.

Participant Activity Survey for Subawardees (PAS-SUB)

The Participant Activity Survey (PAS) is a standard, anonymous online survey that collects process improvement data from participants in activities that are both NNLM-funded and subawardee-led.

Additional Evaluation Resources / Recursos de Evaluación Adicionales

  • Copy of PAS-SUB Survey Instrument:  available in English and en español 
    • These PDFs are not intended for data collection. Please contact your NNLM Coordinator to discuss options for administering this survey.
    • This version of the PAS-SUB includes space for activity-specific learning objectives (see question #2); these learning objectives are optional.
  • Developing S.M.A.R.T. Objectives (2-page resource):  available in English and en español (para crear objetivos SMART, use este enlace)

Reporting Mechanisms for Professional Development Awards and Technology Awards (Final Reports)

All NNLM-funded projects have reporting requirements. Drupal Reporting System reports already exist that require descriptive information about your project.  In addition to these reports, NNLM-funded projects may be required to provide specific information for the following two types of activities:  Professional Development and Technology.

If you are applying for NNLM funding for these activities, you will need to describe in your evaluation plan how and when you intend to collect the required information and any additional evaluation that you are planning to do for your project. 

Please work closely with your NNLM Regional Medical Library or Office to determine which forms you will need to use.

 

NNLM IDs

Many of our questionnaires ask for an NNLM ID. To find your organization's NNLM ID,

  1. Look up your member organization in the Member Directory.
  2. Your NNLM ID will be the first entry on your member organization's directory page.

 

Professional Development

If you are an individual who has received funding from NNLM to attend a professional development activity such as a conference or online training, please follow the instructions below. If you are applying for NNLM funding, take a look at this form to know what data you will be gathering as part of your project.  Note that this funding is not for organizations, only for individuals. If your funding includes purchasing technology, you will also need to fill out the Technology Report.

Recipients of Professional Development funding from the NNLM, please complete the following report after completion of your activity: Professional Development Award Report. You will need your NNLM ID for this form. Your NNLM ID will be the first entry on your member organization's directory page.

Technology Report

If you have received a sub-award from the NNLM that includes purchasing technology, please follow the instructions below. If you are applying for NNLM funding, take a look at this form to know what data you will be gathering as part of your project.

If technology is provided to your organization through funding from an NNLM sub-award, complete this report by the end of your subaward period: Technology Report. You will need your NNLM ID for this form. Your NNLM ID will be the first entry on your member organization's directory page.
