Funds from this award will be used to attend the Medical Library Association Conference in Seattle on May 26-31, 2017. A poster abstract I submitted was accepted and I would like to attend to present the poster. The poster represents a health and wellness community collaborative developed and implemented at the Munson Community Health Library in Traverse City, Michigan. Initially, the Knowledge Management Services Department and the Community Health Library, worked collaboratively with the Grand Traverse County Senior Center Network to bring health and wellness resources to senior citizens in the Grand Traverse region. The program design focused on providing bundled health information in a tote bag. The Health & Wellness To Go Community Collaborative was launched to meet the needs of this under served population. Funding was provided by the GMR through a Community Engagement Award.
