Express Outreach Projects: Report Guidelines

Submission Procedures

  • Quarterly reports must be submitted within 30 days after the quarter ending date.
  • Submit Quarterly reports, Outreach forms, and other attachments as e-mail attachments to Lori Tagawa at ltagawa@library.ucla.edu or to the general PSR mailbox at psr-nnlm@library.ucla.edu

Cover Sheet

The cover sheet for the report (which should be on a separate page from the actual report) should include:

  • Your project name
  • Name of institution
  • Location of institution
  • Purchase order number
  • Name, address and telephone number of person submitting report
  • Number and inclusive dates of report
  • Date submitted

Outline for Quarterly Reports

  • Success Stories: Include your successes with the target population as well as successes they have had in searching for or obtaining needed information.
  • Impact of Information: Incidents from your target population relating the use of the information they have obtained or training they have received.
  • Problems: Include any problems you have had carrying out the project or any problems encountered by your target population in obtaining biomedical information. If a resolution was found please include.
  • Administrative Activities: Include any activities that did not fall under the categories above.
  • Activities Planned for Next Quarter: Describe upcoming activities.

Outreach Reporting

The Outreach Reporting Forms and Participant Information Sheets must be completed for each activity involving training sessions, presentations, or demonstration sessions conducted during the quarter. Copies of these forms are available online from the NN/LM PSR web site. You may also get an account to do online reporting of outreach activity data.

Other Attachments

Attach copies of materials produced, including training materials, press releases, advertisements, articles for newsletters, brochures, bookmarks, etc. (Send a print copy of any item not available in electronic format.)

Final Report

At the end of the project, a final report must be submitted. No quarterly report will be required for the period when the final report is due. The final report should include:

  • A summary of the project's accomplishments
  • An analysis and conclusions of evaluation/outcome data measurements
  • Any issues or barriers involved in completing the project
  • Recommendations for improving project outcomes in the future
  • Impact of the project and any follow-up activities planned
