NLM is no longer providing copies of Cochrane documents (see message from them below), so the best thing to do is to go directly to the source, Cochrane itself. They charge $15 to mail or fax copies, see

NLM's ILL Customers,
As of Monday May 6, 2002, NLM is suspending delivery of ILL requests for Cochrane Library documents pending resolution of outstanding licensing issues. NLM will post another message once these issues have been resolved. If you have a request pending (Received) with NLM for a Cochrane document, you can either phone us with the DOCLINE request number to cancel it now (301-496-5511), or wait until we know more. You may also contact Cochrane (http://www.cochranelibrary.com/enter/) directly to order copies of any documents.

NLM Resumes ILL for Cochrane Library Documents

As of Wednesday afternoon, May 8, 2002, NLM is resuming
delivery of ILL requests for Cochrane Library documents. Any
documents in the "receipt queue" at NLM since Monday May 6,
should be prepared for delivery later today or tomorrow
morning. We also encourage users to contact Cochrane directly
at:
http://www.cochranelibrary.com/document_delivery/Doc-delivery.html
to
order copies of any documents.
