The National Partnership for Action to End Health Disparities released a Toolkit for Community Action to support community efforts to eliminate health disparities and promote health equity. The toolkit aims to:
- Raise awareness about health disparities
- Engage others in a conversation about the problems and soluttions
- Take action for change
The tooklit provides facts and tips to learn about health disparities and social factors; to provide culturally and linguistically competent care; and to learn about how racial and ethic groups can benefit from the Affordable Care Act. Use the toolkit to increase awareness about health disparities, become a leader for addressing health disparities, support healthy behaviors in your community, improve access to health care, and create healthy neighborhoods. This toolkit is useful for librarians interested in health information outreach and community engagement.
